Printable Group Order Form

Our group order form can simplify the process of ordering for a large group of people. Use our catalog creation to print out the designs you want to offer to your group. You can select only one design for a uniform look, all designs for the most selection, or any number in between. Churches may want to choose one design for children, one for the youth, one for the choir, etc.

The printed format makes it easy for you to show your selected designs to your church or group members. The order form allows you to keep track of names and phone numbers, so that when you receive your order, you will know exactly who ordered what.

When your order form is completed, simply mail or fax it back to us. If you prefer to order online, you can also use your printed sheet as a reference to enter your order in our secure shopping cart, and still have a record of individual orders.

After printing, you are encouraged to complete your order as soon as possible, as design availability and pricing are subject to change. If there is a problem with your order, we will make every effort to contact you, so please be sure to complete the entire form.
